You can find a detailed explanation of the needed information in our Data Request and Cash Flow lists. You can submit these documents in one of two ways, please choose the option that is most convenient for you.
1. Electronically (preferred): Click the link in the email you received from Sequoia’s Wealth Planning Department or your advisor, which will prompt you to enter your email address and a PIN contained in that same email communication. If you did not receive this information, please contact us at email@example.com. Once you start the form you will be able to use your login credentials to access it again in the future if you don’t complete the data request in one sitting. This electronic form will allow you to securely provide Sequoia with electronic copies of your statements and/or provide you the ability to key in financial information and notes.
2. Printed Forms: Click the linked PDF and complete accordingly. Once complete, please mail to ATTN: Wealth Planning Dept, 3500 Embassy Parkway, Akron, OH 44333 or submit electronically using our secure SendThisFile service.
